The three Medicaid programs you'll hear about
NJ FamilyCare
The umbrella program most kids are on. Covers doctors, hospitals, dental. Renews yearly.
Medicaid ABD
For Aged, Blind, and Disabled adults with limited income. Common path at age 21.
DDD-Medicaid (SED/CCP/SPP)
The waiver that funds DDD services once eligibility is approved.
The age-21 transition (the biggest cliff)
At 21, your child leaves family Medicaid and re-applies as an individual. Because DDD looks only at their income and assets, most people qualify even when parents earn well above the family limit.
Start 6 months before their 21st birthday. Gaps in Medicaid = gaps in DDD services. Contact your County Board of Social Services and request "Community Medicaid / MLTSS-DDD."
